IMO: the nipple/cheek area needs some kind of shadow or depth. Now it looks not real. Other parts in the image as well need more depth.

I see what you're saying SH. I believe it's the sharpness contrast between the nipple and the distortion blur of the speaker fabric. Gives it an odd floating impression. Some sharp edges may need a bit of softening as well.

SH, light's coming from above and wouldn't cast a shadow on baby's face, I tried it and didn't like anyway.
